A home infusion clinical pharmacist plays a vital role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients in a dynamic healthcare environment. This position requires strong expertise in sterile compounding, clinical monitoring, and patient education while collaborating with a multi-disciplinary team to ensure optimal care.
Schedule:
Monday to Friday, 8:00 AM
  • 4:30 PM Participation in on-call rotation required
Key Responsibilities:
Sterile Compounding:
Prepare specialty pharmaceuticals and IV solutions in compliance with all regulatory standards in a ventilated sterile environment.
Clinical Monitoring:
Evaluate lab results, perform pharmacokinetic dosing calculations (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and provide patient counseling on specialty medications.
Patient Education:
Train patients and caregivers on infusion pumps, sterile techniques, and catheter care to ensure proper home infusion practices.
Care Coordination:
Work closely with physicians, home health nurses, and pharmacy teams for seamless therapy management and reduction of hospital readmissions.
Regulatory Compliance:
Adhere to State Board of Pharmacy rules and other relevant standards such as HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
Inventory Management and Shipping:
Oversee pharmacy inventory including refrigerated medications and coordinate secure delivery to patients.
Desired Experience and Qualifications:
Minimum 2 years of infusion specialty pharmacy experience IVIg experience considered a plus Familiarity with specialty therapy payer requirements and reimbursement processes Current, active NY pharmacist license in good standing Experience with CareTend system preferred

Liverpool, NY This is a direct hire position with a compensation range of $124,800 to $145,600, depending on experience.
