Job Overview Join our dynamic team as a Pharmacist at the MTM (Medication Therapy Management) Center of Excellence, working Monday through Thursday from 9:00 AM to 4:30 PM. In this vital role, you will provide expert pharmaceutical care, optimize medication use, and ensure regulatory compliance within a fast-paced healthcare environment. Your expertise will directly impact patient outcomes by delivering comprehensive medication management services, collaborating with healthcare providers, and supporting innovative pharmacy practices. This position offers an exciting opportunity to utilize your clinical skills and advance patient-centered care in a collaborative setting. Responsibilities Conduct thorough medication reviews and patient assessments to identify therapy optimization opportunities. Provide medication counseling and education tailored to diverse patient populations, including pediatrics and oncology cases. Collaborate closely with healthcare teams to develop and adjust medication plans, ensuring safe and effective therapy. Maintain accurate documentation within Electronic Medical Records (EMR/EHR) systems, ensuring compliance with pharmaceutical regulations. Monitor for adverse drug reactions, drug interactions, and contraindications, especially in critical care settings such as ICU or emergency room environments. Ensure aseptic techniques during IV infusion preparation and compounding, adhering to strict hospital pharmacy standards. Stay current with pharmaceutical regulatory updates and implement best practices for medication safety and compliance. Skills Extensive knowledge of pharmacology, pharmacotherapeutics, physiology, and medical terminology necessary for comprehensive patient care. Proven experience in hospital pharmacy settings, including hospital medicine, critical care, ICU, PICU (Pediatric Intensive Care Unit), or Level I/II trauma centers. Strong background in managed care principles and medication regulation compliance. Skilled in EMR/EHR systems and Pharmacy Information Systems (PIS) for accurate documentation and medication management. Experience with IV compounding, aseptic techniques, pain management, oncology medications, and emergency room procedures. Demonstrated ability to practice as a licensed pharmacist with post-residency or post-training experience in acute care environments. Excellent patient service skills with the ability to perform detailed patient assessments and medication administration oversight. Knowledge of hospital pharmacy operations including pharmacy regulatory standards and pharmaceutical regulatory compliance requirements. Ability to precept pharmacy students or residents as part of educational initiatives.
