This position acts as the central coordinator within a multidisciplinary team, ensuring safe sterile compounding, precise medication adjustments, and thorough patient education.

Direct Hire Core Responsibilities:

Prepare specialty pharmaceuticals and IV solutions under sterile, ventilated conditions, complying with all state and federal regulations. Assess laboratory results, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize treatments. Counsel patients and caregivers on specialty medications, infusion pump operation, sterile techniques, and catheter maintenance. Collaborate with prescribing physicians, home health nurses, and internal teams—including intake, referral, reimbursement, nursing, clinical services, and pharmacy operations—to enhance therapy management and reduce hospital readmissions. Manage compliance with regulatory standards, including those set by State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Oversee inventory management, including refrigerated products, and ensure safe, timely packaging and shipping of medications to patients' homes. Qualifications /

Desired Experience:

Minimum 2 years of infusion specialty pharmacy experience. Experience with IVIg therapy is a plus. Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations. Current and in good standing NY pharmacist license.
