Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A clinical pharmacist specializing in home infusion plays a crucial role in delivering complex intravenous (IV) and subcutaneous medications to patients in a safe, effective, and patient-centered manner. Situated in Dublin, OH, this direct hire opportunity is ideal for professionals seeking to apply their infusion specialty pharmacy expertise in a dynamic and collaborative environment.
Key Responsibilities:
Sterile Compounding:
Prepare specialty pharmaceuticals and IV solutions within a sterile, ventilated setting, adhering strictly to state and federal regulations.
Clinical Monitoring:
Evaluate laboratory results, perform pharmacokinetic dosing calculations (such as vancomycin), and monitor Total Parenteral Nutrition (TPN) to tailor patient treatments. Provide thorough counseling and education about specialty medications and treatment regimens.
Patient Onboarding & Education:
Instruct patients and caregivers on the correct use of infusion pumps, sterile techniques, and catheter maintenance at home to promote safe self-administration.
Care Coordination:
Work closely with prescribing physicians, home health nurses, and cross-functional teams—including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations—to ensure continuous, high-quality therapy and reduce hospital readmission rates.
Regulatory Compliance:
Maintain full compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards.
Inventory & Shipping Management:
Oversee pharmacy inventory with a focus on refrigerated products and prepare medications and supplies to guarantee safe and timely delivery to patients.
Qualifications:
Minimum 2 years of infusion specialty pharmacy experience Experience with IVIg therapies is a plus Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations Current and good standing Ohio pharmacist license Familiarity with CareTend system Candidates with a passion for clinical excellence, patient education, and teamwork will thrive in this role.
