Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire An experienced clinical pharmacist with IV certification is needed to oversee complex home infusion therapies in a direct hire role. This position requires a detail-oriented professional who will prepare and monitor intravenous (IV) and subcutaneous medications, ensuring patient safety and optimal therapeutic outcomes. The role serves as a key member of a multidisciplinary team dedicated to the highest standards of home infusion pharmacy care.

Monday - Friday, 8:30 AM - 5:00
PM On Call:
Participate in rotation for after-hours support

ASAP Responsibilities:
Prepare sterile compounded specialty pharmaceuticals and IV solutions in compliance with federal and state regulations. Monitor clinical parameters including lab values and pharmacokinetic dosing (e.g., vancomycin), managing Total Parenteral Nutrition (TPN) regimens. Educate patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance to support safe home administration. Collaborate with prescribing physicians and home health nurses to coordinate care and reduce hospital readmissions. Maintain compliance with regulatory standards including State Board of Pharmacy, HIPAA, DEA, Medicare, and others. Manage pharmacy inventory, including refrigerated medications, and coordinate shipping to ensure timely delivery. Work closely with intake, referral, reimbursement, nursing, clinical, and pharmacy operations teams to enhance patient care continuity.
Qualifications:
Minimum 2 years of infusion specialty pharmacy experience required. IV Immunoglobulin (IVIg) experience is a plus. Familiarity with specialty therapy payer requirements, reimbursement procedures, and contract obligations. Current, active Ohio pharmacist license in good standing.
