Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire A dynamic opportunity is available for a home infusion clinical pharmacist seeking a direct hire position with immediate start. This role is based Monday through Friday from 8:30 a.m. to 5:00 p.m., including participation in an on-call rotation. The position requires hands-on expertise in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, playing a vital role within a multidisciplinary healthcare team.
Key Responsibilities:
Sterile Compounding:
Accurately prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment, strictly adhering to all state and federal regulations.
Clinical Monitoring:
Evaluate lab results, calculate pharmacokinetic dosing (such as vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatments. Provide thorough patient counseling on specialty medications and treatment plans.
Patient Education:
Instruct patients and caregivers on infusion pump operation, sterile techniques, and home catheter care to ensure safe and effective therapy.
Care Coordination:
Collaborate closely with prescribing physicians, home health nurses, and internal teams—including intake, referral, reimbursement, nursing, clinical services, and pharmacy operations—to ensure seamless care and reduce hospital readmissions.
Regulatory Compliance:
Maintain strict compliance with regulatory requirements from State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other standards.
Inventory and Shipping:
Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for safe and timely delivery to patients' homes.
Qualifications:
Minimum of 2 years' experience in infusion specialty pharmacy IVIg experience considered a plus Thorough understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations Current and active Ohio pharmacist license in good standing Familiarity with CareTend system is preferred
