Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire A home infusion clinical pharmacist role is available as a direct hire position in Hilliard, OH. This position offers a Monday through Friday schedule from 8:30 a.m. to 5 p.m., with participation in an on-call rotation. The ideal candidate will have specialized experience in sterile compounding and clinical monitoring within a home infusion setting. This role serves as a critical link within a multidisciplinary team, providing expert preparation of intravenous (IV) and subcutaneous medications, and ensuring patients and caregivers receive essential education and support.
Responsibilities:
Prepare specialty pharmaceuticals and sterile IV solutions in compliance with state and federal regulations. Assess patient lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN), adjusting treatments accordingly. Provide detailed patient counseling and education on specialty medications and treatment protocols. Train patients and caregivers on infusion pump usage, sterile technique, and catheter maintenance. Collaborate with prescribing physicians, home health nurses, and internal teams to promote optimal therapy management and reduce hospital readmissions. Ensure adherence to regulatory standards including the State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Oversee pharmacy inventory management, including temperature-sensitive products, and prepare medication shipments for safe delivery to patients.
Qualifications:
Minimum of 2 years' experience in infusion specialty pharmacy. IVIg experience is considered a plus. Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations. Current and valid Ohio pharmacist license in good standing.
