Category Pharmacy - Pharmacist Job Type Contract Provide expert pharmacy care across multiple hospital sites in the Greater Cleveland area by filling a PRN pharmacist role. This position offers opportunities to exercise clinical expertise, medication management, and patient counseling on a flexible, as-needed basis. The assignment begins ASAP and lasts approximately 13 weeks.

Core Responsibilities:

Complete medication reconciliation and create accurate prior-to-admission medication lists using multiple information sources. Deliver discharge medication counseling to patients and families. Monitor drug therapy for contraindications, interactions, allergies, and proper dosing. Provide clinical consultation and medication recommendations to healthcare providers. Participate in antimicrobial stewardship, pharmacokinetic dosing/monitoring, and anticoagulation dosing/monitoring activities. Ensure medication inventory accuracy, prepare and dispense medications, verify orders, and maintain controlled substance accountability. Support quality assurance, medication-use reviews, and Pharmacy & Therapeutics Committee activities. Maintain compliance with federal and state pharmacy regulations and facility policies. Offer staff education, new employee training, and serve as a resource within the pharmacy team.

Desired Qualifications:

Active Ohio Pharmacist license required. Hospital/inpatient pharmacy experience strongly preferred. Availability to work PRN day shifts with willingness to travel between Cleveland-area sites. Experience with EPIC electronic medical records preferred. Strong clinical knowledge and communication skills. Proficiency in medication reconciliation, antimicrobial stewardship, pharmacokinetic and anticoagulation dosing.
