Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A clinical pharmacist specializing in home infusion pharmacy plays a pivotal role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients in a home care setting. This position requires a strong clinical focus and the ability to collaborate effectively with a multidisciplinary healthcare team to ensure optimal patient outcomes.
Location:
Westerville, OH Responsibilities:
Sterile Compounding:
Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment compliant with state and federal regulations.
Clinical Monitoring:
Evaluate laboratory values, calculate pharmacokinetic dosing (such as vancomycin), and monitor Total Parenteral Nutrition (TPN) therapies. Provide patient counseling and education on specialty medications and treatment plans.
Patient Onboarding & Education:
Train patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance at home.
Care Coordination:
Work collaboratively with prescribing physicians, home health nurses, and various internal teams including intake, reimbursement, nursing, clinical services, and pharmacy operations to ensure smooth therapy management and reduce hospital readmissions.
Regulatory Compliance:
Ensure adherence to State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
Inventory & Shipping:
Manage pharmacy inventory including refrigerated products and oversee the safe preparation and shipping of medications and supplies to patients' homes.
Qualifications:
Minimum of 2 years' experience in infusion specialty pharmacy. IVIg experience is highly desirable. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations. Current and active pharmacist license in New York in good standing.
