In this vital role, you will provide expert pharmaceutical care, ensuring safe and effective medication management for diverse patient populations. Duties Dispense prescriptions accurately while adhering to all legal and safety regulations, including HIPAA compliance. Conduct comprehensive patient assessments to determine appropriate medication therapy and provide counseling on medication use, side effects, and interactions. Collaborate with healthcare providers on medical management plans, including pain management, immunizations, and IV infusions. Manage inventory efficiently by ordering medications, compounding sterile and non-sterile medications, and maintaining proper stock levels. Utilize pharmacy software, EMR (Electronic Medical Records), and EHR (Electronic Health Records) systems to document patient interactions and medication records precisely. Perform aseptic techniques during compounding medications and IV insertions to ensure patient safety. Provide specialized care in LTC (Long-Term Care), critical care, pediatrics, and acute care settings by applying physiology knowledge, medical terminology expertise, and anatomy understanding. Assist with patient service initiatives such as medication administration education, immunizations, and patient care planning. Support pharmacy technician staff in daily operations to ensure smooth workflow and high-quality patient care. Qualifications Valid pharmacy license with current registration in the relevant jurisdiction. Proven experience in LTC pharmacy, critical care environments, or hospital settings is highly desirable. Completion of a pharmacy residency program or equivalent clinical pharmacy experience preferred. Strong knowledge of medical terminology, physiology, anatomy, and medication management protocols. Hands-on experience with pharmacy software systems, EMR/EHR platforms, and inventory management tools. Skilled in compounding medications using aseptic techniques and performing IV insertions with precision. Knowledge of immunizations administration procedures and HIPAA regulations for patient privacy protection. Experience in pain management protocols and acute care procedures is advantageous. Pharmacy technician experience is a plus for supporting daily operations efficiently.
