This role requires a professional with strong pharmacy skills, including IV certification and expertise in staff pharmacist responsibilities within a home infusion setting.
Responsibilities:
Accurately prepare, dispense, and review medications, focusing on home infusion therapies. Ensure compliance with all regulatory requirements and safety standards. Collaborate with healthcare providers to optimize patient outcomes. Maintain detailed and precise patient documentation. Provide counseling to patients and caregivers regarding medication use and infusion procedures. Monitor inventory and manage supplies related to infusion services. Participate in quality assurance programs and continuous professional development.
Qualifications & Desired Experience:
Active pharmacist license and credentials relevant to state regulations. IV certification required, demonstrating competency in sterile compounding and intravenous therapies. Proven experience as a staff pharmacist, particularly within home infusion pharmacy settings. Strong knowledge of pharmacology, drug interactions, and infusion techniques. Excellent communication and patient care skills. Ability to work independently and as part of a multidisciplinary team.
