Hood River, OR Job Description We are seeking an experienced Outpatient Pharmacist for a travel assignment in a critical access healthcare facility. The ideal candidate will have outpatient infusion experience and strong clinical pharmacy skills in an outpatient setting. First-time travelers with the required experience are welcome to apply.

Key Responsibilities:

Provide pharmaceutical care and medication management in an outpatient setting Support outpatient infusion therapy services Review medication orders for accuracy, safety, and compliance Counsel patients regarding medications, dosages, and treatment plans Collaborate with physicians, nurses, and interdisciplinary healthcare teams Maintain accurate documentation within the EMR system

Requirements:
