Job Summary AidRx is seeking a Pharmacist to join our advanced LTC pharmacy team. The ideal candidate is accurate, clinically strong, detail-oriented, and comfortable in a fast-paced environment with digital systems and medication automation. This role requires both clinical judgment and operational precision, supporting patients who rely on consistent and well-managed medication therapy as part of their ongoing care. Responsibilities Dispense and verify prescriptions for home-based LTC patients with a high level of accuracy and clinical responsibility Perform full clinical and product verification in accordance with pharmacy standards and best practices Review orders for accuracy, completeness, and therapeutic appropriateness, identifying and resolving potential issues Conduct patient medication assessments and monitor adherence, helping ensure optimal therapeutic outcomes Provide clear and professional medication counseling to patients and caregivers Communicate effectively with nurses, caregivers, and providers regarding medication needs, changes, and clarifications Work with QS/1, DocuTrack, and EHR systems to ensure accurate documentation and workflow efficiency Operate and oversee Parata automation and compliance packaging systems, ensuring accuracy and consistency Maintain controlled substance documentation and ensure full regulatory compliance Participate in quality assurance initiatives and contribute to continuous process improvement within pharmacy operations

Bachelor's or Doctor of Pharmacy (PharmD) Active state pharmacist license LTC, clinical, or hospital pharmacy experience preferred Minimum 5 years LTC/independent pharmacy experience strongly preferred Proficiency with QS/1 and DocuTrack Experience with Parata automation preferred Strong knowledge of drug interactions, clinical protocols, and pharmacy regulations Excellent communication and patient-care skills, particularly when working with chronic care populations Ability to work efficiently in a fast-paced, high-volume, and technology-driven environment Knowledge of geriatric pharmacotherapy and MTM is a plus EHR experience preferred Physical Requirements Ability to stand or sit for 6-8 hours Ability to lift up to 50 lbs. as needed Occasional bending, kneeling, or climbing
