Key Responsibilities:

Evaluate, prepare, and dispense medications accurately in compliance with professional standards, facility protocols, and state and federal regulations. Conduct medication reconciliation processes when applicable to ensure seamless continuity of care for patients upon discharge. Provide outstanding customer service and maintain a focus on clinical care excellence. Consult with patients regarding prescribed medications and supplies, including dosage instructions, frequency, special requirements, and possible drug interactions.

Desired Qualifications:

Valid Pennsylvania Pharmacist License. Experience working in a closed-door pharmacy setting preferred. Strong attention to detail and dedication to patient safety and care. Excellent communication skills with patients and healthcare team members.
