is currently seeking a full-time Clinical Pharmacist. The Clinical Pharmacist will work with providers, care managers, and other clinical staff as part of a medical care team focusing on improving patient outcomes, while maintaining cost. The Clinical Pharmacist provides patients and providers with therapeutic assessments, tools to improve medication management, and an extensive knowledge base to promote the mission and values of the high-quality patient care. Responsibilities include, but are not limited to:

• Developing and implementing protocols for patients and providers to optimize medication regimens
• Develop cost-reduction strategies and provide therapy optimization recommendations to provider offices for patient medication affordability
• Collaborate with other members of the care team to reach pharmacy-related quality metric goals and provide enhanced patient care support
• Support population health initiatives, such as Transitional Care Management, Chronic Care Management, and Remote Patient Monitoring
• Provide education to providers and staff via presentation, newsletters, formal utilization reviews, and one-on-one interactions
• Provide office-based, hands-on consultative workflow transformation and training/education services to PCPs and facilities enrolled in the Organization's value-based reimbursement programs.

Assess the needs of each individual PCP practice and use a combination of on-site individual, office-based, entity-level-based interactions to drive engagement and exemplary value through cost control, clinical pathway development, and ROI/fiscal outcomes as set by the Organization for PCP and facility programs Must have an active Pennsylvania pharmacist license Must have at least 2 years' experience in clinical pharmacy with involvement in direct patient counseling and management, medication adherence, and pharmacy/insurance claims processing Must be able to work independently exercising sound judgement, discretion, and the initiative to facilitate change and have excellent analytical, problem-solving abilities, interpersonal, organizational, written, and verbal skills.
