Job Description

We are seeking a
PART TIME
Staff Pharmacist to provide comprehensive clinical pharmacy services and support safe, effective medication use throughout the hospital. This position works closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. Key Responsibilities Perform daily duties as a staff pharmacist while providing clinical pharmacy services. Review patient medication regimens and clinical data to identify drug interactions, incompatibilities, dosing concerns, and other medication-related issues. Consult with prescribers, nurses, and other healthcare professionals regarding medication therapy. Calculate and recommend appropriate medication doses, including pharmacokinetic dosing, for patients of all ages. Interpret laboratory values, patient age, weight, and other clinical information to determine appropriate medication dosing. Provide drug information and medication-related education to patients and healthcare professionals. Participate in multidisciplinary meetings and patient care teams related to medication management. Supervise pharmacy technicians and ensure pharmacy tasks are completed accurately and appropriately. Participate in formulary management, including new medication reviews, drug class reviews, therapeutic interchange, and recommendations for additions or deletions. Maintain and support the effective use of drug information resources. Assist with the development, review, implementation, and maintenance of pharmacy policies and procedures. Monitor and analyze adverse drug reactions (ADRs), including preventability and severity, and communicate findings to pharmacy leadership and the Pharmacy & Therapeutics Committee. Report ADRs to the FDA when appropriate and incorporate findings into quality improvement initiatives. Conduct drug utilization evaluations and communicate findings and recommendations for process improvement. Develop and participate in quality improvement initiatives related to medication management. Participate in pharmacy staff education and competency assessments on clinical and medication-related topics. Assist with Pharmacy & Therapeutics Committee activities, including new drug reviews and pharmacy newsletters. Ensure medications are used safely, accurately, and appropriately for patients of all ages in accordance with professional standards, hospital policies, and pharmacy procedures. Minimum Qualifications Bachelor of Science in Pharmacy or Doctor of Pharmacy (Pharm.

D.) degree. Current, valid Pennsylvania pharmacist license
REQUIRED
Knowledge of hospital pharmacy practice, medication therapy, drug distribution, and medication control processes. Ability to interpret medication orders and clinical information accurately. Ability to calculate medication doses and apply pharmacokinetic principles. Ability to interpret laboratory values and other patient-specific information when evaluating medication therapy. Proficiency with computers and pharmacy technology, including automated dispensing cabinets and other pharmacy equipment. Ability to work effectively as part of a multidisciplinary healthcare team. Strong communication, clinical judgment, organizational, and supervisory skills. Work Environment & Schedule This position works in a contemporary hospital pharmacy environment providing unit-dose medication distribution, IV admixture services, drug information, and clinical pharmacy services using computerized pharmacy systems. Pharmacist coverage is provided 7:00 a.m.-5:00 p.m., Monday through Friday , and 7:00 a.m.-3:30 p.m., Saturday, Sunday, and holidays . Shift hours may vary based on departmental coverage needs. Regular after-hours call coverage is required. Pharmacists must be able to return to the hospital within one hour of receiving a call-back request.
Pay:
