About Corewell Health Grand Rapids Hospitals Retail Pharmacy Corewell Health Grand Rapids Hospitals Retail Pharmacy is a closed-door, outpatient pharmacy located at Butterworth Hospital in Grand Rapids, Michigan. This pharmacy operates under a "Specialty-Plus" Model, dispensing both Specialty and Mon-Specialty Medications pertinent to specific disease states that often require a higher level of care. This frequently involves a multidisciplinary, collaborative approach to ensure proper medication coordination and administration. Patient populations currently served include Hemophilia and Cystic Fibrosis. //www.spectrumhealth.org/locations/butterworth-hospital Scope of Work Provides pharmaceutical care according to professional and departmental standards and statutory requirements. Provides safe, effective, rational, and cost-conscious pharmacotherapeutic recommendations through the department's medication management programs. The Clinical Pharmacist assists in supervising the functional activities of Pharmacy Technicians and Interns. The clinical pharmacist has state of Michigan pharmacist in charge responsibilities for this pharmacy location including compliance to all regulatory requirements. Maintains accountability for ensuring the safety and efficacy of medication use. Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations. Works with multiple medication automation systems. Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions. Safely compounds and dispenses medications for administration to patients. Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy. Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel. Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis. Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications. Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements. Qualifications Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy 2 years of relevant experience hospital pharmacy Preferred American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred LIC-Clinical Pharmacist
