In this vital role, you will provide expert pharmaceutical care to patients across various clinical settings, ensuring safe and effective medication use. Your responsibilities will include collaborating with healthcare professionals, educating patients on medication management, and maintaining accurate medical documentation. The ideal candidate will possess a strong foundation in pharmacy practices, excellent communication skills, and a passion for improving patient outcomes through meticulous pharmaceutical care. Duties Dispense medications accurately while verifying prescriptions against patient medical records and treatment plans Collaborate with physicians, nurses, and other healthcare providers to optimize medication therapy and ensure patient safety Provide health education to patients regarding medication usage, potential side effects, and interactions Monitor patient conditions closely, reporting any adverse reactions or changes in health status promptly Manage electronic health records (EHR) efficiently, ensuring all medical documentation is complete, accurate, and up-to-date Certified in Cardiopulmonary Resuscitation (CPR) Maintain compliance with medical terminology standards and manual handling protocols during medication distribution Support nursing staff and doctors in medication administration Engage in patient service activities including counseling on medication adherence and lifestyle modifications Experience Proven experience as a Clinical Pharmacist Familiarity with electronic health records (EHR) systems and medical documentation standards Knowledge of medical terminology relevant to pharmacy practice and patient care protocols Certification in Basic Life Support (BLS) and CPR is required; additional certifications provided by BJVIM Prior experience working with multidisciplinary teams including nurses and doctors Join us to make a meaningful difference in patient care through expert pharmaceutical services!
