```- Review and interpret prescriptions to ensure accuracy and appropriateness
- Dispense medications and provide medication counseling to patients
- Collaborate with healthcare professionals to develop and implement medication therapy plans
- Monitor patient responses to drug therapy and make recommendations for adjustments as needed
- Conduct medication reconciliation to prevent adverse drug events
- Provide drug information and education to healthcare professionals, patients, and caregivers
- Participate in quality improvement initiatives and medication safety programs ```
Skills:
``` To excel in this role, you should possess the following skills:- Acute pain management: Knowledge of pain management strategies for acute conditions
- Anatomy knowledge: Understanding of human anatomy and its relevance to medication therapy
Pediatrics:
Familiarity with pediatric pharmacotherapy and dosage adjustments- Acute care: Experience working in an acute care setting, such as a hospital or clinic
Hospital:
Knowledge of hospital pharmacy operations and regulations- Physiology knowledge: Understanding of physiological processes and their impact on drug therapy
- Utilization review: Ability to assess the appropriateness of medication use based on clinical guidelines and formulary restrictions
- Medication administration: Proficiency in preparing and administering medications using various routes (e.g., oral, intravenous)
- Medical management: Familiarity with medical management principles, including disease state management and treatment protocols
