This position ensures the safe and compliant dispensing and compounding of medications, performs clinical assessments, oversees medication therapy management, and supports pharmacy operations in accordance with state and federal regulations, ACHC standards, USP / requirements, and Vital Care policies and procedures. The Staff Pharmacist collaborates closely with prescribers, nurses, patients, caregivers, and pharmacy staff to optimize clinical outcomes and deliver exceptional patient care. This role supports specialty pharmacy, sterile compounding, and infusion services while maintaining compliance with all applicable accreditation and regulatory requirements. Essential Duties and Responsibilities Clinical Services Review, verify, and dispense prescription medications accurately and safely. Conduct comprehensive patient assessments, including medication reconciliation, allergy reviews, clinical appropriateness evaluations, and ongoing therapy monitoring. Develop and maintain patient-specific care plans for specialty and infusion therapy patients. Monitor patients for medication effectiveness, adverse drug reactions, drug interactions, and compliance concerns. Provide clinical consultation and drug information to physicians, nurses, patients, and caregivers. Perform patient counseling regarding medication administration, storage, side effects, adherence strategies, and expected therapeutic outcomes. Participate in on-call pharmacist responsibilities as required. Specialty and Infusion Pharmacy Services Review infusion orders for clinical appropriateness and compliance with established treatment protocols. Support specialty pharmacy programs through medication therapy management, adherence monitoring, refill coordination, and clinical follow-up. Collaborate with nursing staff to address infusion-related concerns, adverse events, and therapy modifications. Ensure safe handling, preparation, labeling, storage, and dispensing of specialty medications and sterile compounded preparations. Sterile Compounding and Regulatory Compliance Supervise and verify sterile compounding activities in accordance with

, state board regulations, and company policies. Verify compounding records and perform final product verification. Ensure compliance with ACHC accreditation standards and pharmacy quality assurance programs. Participate in environmental monitoring, competency evaluations, quality improvement initiatives, and performance audits. Maintain complete and accurate documentation within pharmacy management, EHR, and dispensing systems. Pharmacy Operations Oversee daily workflow to ensure timely and accurate medication preparation and dispensing. Supervise and support pharmacy technicians, interns, and ancillary staff. Assist with inventory management, medication purchasing, storage, recalls, and controlled substance accountability. Ensure compliance with HIPAA, OSHA, DEA, state board regulations, and company policies. Participate in continuous quality improvement, risk management, and patient safety initiatives. Assist with accreditation surveys, regulatory inspections, and audit preparation. Qualifications Required Doctor of Pharmacy (PharmD) Active pharmacist license in Tennessee or ability to obtain licensure before employment. Minimum one (1) year of pharmacist experience or completion of relevant pharmacy training. Knowledge of federal and state pharmacy laws and regulations. Strong clinical decision-making and patient care skills. Proficiency with pharmacy software systems, electronic health records, and Microsoft Office applications. Excellent communication, organizational, and interpersonal skills. Preferred Previous experience in specialty pharmacy, infusion pharmacy, home infusion, hospital pharmacy, or sterile compounding environments. Experience with ACHC, URAC, or other accreditation standards. Knowledge of USP and USP requirements. Current Basic Life Support (BLS) certification. Physical Requirements Ability to stand and walk for extended periods. Ability to lift and move packages or supplies weighing up to 25 pounds. Ability to work in cleanroom and sterile compounding environments while wearing required personal protective equipment.
