Responsibilities Dispense prescription medications accurately using advanced pharmacy software systems, ensuring compliance with all legal and safety standards. Conduct thorough patient assessments, including medication history reviews, to develop personalized treatment plans. Collaborate with healthcare providers to optimize medication therapy, especially in specialized areas such as LTC (Long-Term Care), critical care, pediatrics, and acute care settings. Prepare and compound medications following aseptic technique and compounding guidelines to ensure safety and efficacy. Administer immunizations and provide patient education on medication use, potential side effects, and health management strategies. Manage inventory levels efficiently, including ordering, stock rotation, and proper storage of pharmaceuticals. Document all patient interactions and medication records accurately within Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems while maintaining strict adherence to HIPAA regulations. Assist in medical management initiatives such as pain management protocols and IV infusion therapies, including IV insertion and aseptic procedures. Support pharmacy technician staff by overseeing workflow, verifying prescriptions, and ensuring quality control in medication preparation. Stay current with medical terminology, physiology knowledge, anatomy fundamentals, and updates in pharmacy practice to provide informed patient care. Experience Proven experience working with a strong background in pharmacy practice. Experience is highly desirable for managing complex medication regimens in high-pressure settings. Prior pharmacy residency or clinical pharmacy experience that demonstrates advanced knowledge of pharmacology and patient management is desired but not required Familiarity with LTC pharmacy operations prefered. Specialized compounding techniques preferred. Immunization administration required Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality at all times. Experience working with pharmacy technicians to coordinate workflow efficiently while ensuring accuracy in dispensing medications.
