Staff Pharmacist - Houston Methodist. The Staff Pharmacist provides safe, evidence-based medication therapy for diverse patient populations within our academic medical system. Responsibilities include order verification, sterile and non-sterile compounding, therapeutic drug monitoring, patient and provider education, and compliance with regulatory and quality standards. Responsibilities Verify and dispense medications ensuring accuracy, safety, and regulatory compliance Perform clinical pharmacotherapy review and therapeutic drug monitoring Compound and supervise preparation of sterile and non-sterile products Provide medication counseling to patients and education to clinicians Collaborate with physicians, nurses, and care teams on medication therapy plans Participate in antimicrobial stewardship and formulary management initiatives Precept and mentor students and residents as assigned Document clinical interventions and outcomes in the electronic health record Support medication safety, quality improvement, and accreditation activities Maintain current knowledge of guidelines, policies, and new therapies Required Skills Medication therapy management Pharmacokinetics and therapeutic drug monitoring Sterile compounding (IV admixture) Non-sterile compounding Electronic health record (EHR) and CPOE systems Interpretation of clinical guidelines and protocols Antimicrobial stewardship Patient and provider medication counseling Regulatory and compliance knowledge (TJC, USP, CMS) Drug information and literature evaluation
