Home Infusion Pharmacist Position Summary The Home Infusion Pharmacist is responsible for the clinical management, preparation oversight, and dispensing of infusion therapies for patients in home and alternate care settings. This role ensures safe, effective, and compliant delivery of medications while collaborating closely with physicians, nurses, and patients to optimize therapeutic outcomes. Key Responsibilities Clinical Care Review, interpret, and verify physician orders for accuracy, appropriateness, and completeness Assess patient-specific factors (labs, allergies, diagnoses) to ensure safe therapy Monitor patient response to therapy and recommend adjustments as needed Provide pharmacokinetic dosing and therapeutic drug monitoring (e.g., vancomycin, aminoglycosides) Collaborate with prescribers and care teams to optimize treatment plans Compounding & Dispensing Oversee or perform sterile compounding in compliance with USP and standards Ensure accurate preparation and labeling of infusion medications Verify compounded products prior to dispensing Maintain quality assurance and infection control standards Patient & Caregiver Education Counsel patients and caregivers on medication use, administration techniques, and potential side effects Provide clear instructions for infusion devices (e.g., pumps, elastomeric devices) Support adherence and answer therapy-related questions Operations & Compliance Ensure compliance with state and federal pharmacy regulations Maintain accurate documentation and patient records Participate in accreditation processes (e.g., ACHC, Joint Commission) Support inventory management, including specialty and high-cost medications Collaboration Work closely with nurses, intake coordinators, and reimbursement teams Communicate therapy changes and clinical updates to the care team Participate in interdisciplinary case reviews Qualifications Education & Licensure Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy Active, unrestricted pharmacist license in the state of practice Experience 1-3+ years of pharmacy experience preferred Home infusion, hospital, or sterile compounding experience strongly preferred Knowledge & Skills Strong knowledge of IV therapies, antibiotics, TPN, and specialty infusions Familiarity with USP and requirements Excellent clinical judgment and attention to detail Strong communication and patient education skills Ability to work independently and in a collaborative team environment Preferred Qualifications Board Certification (e.g., BCPS) Experience with home infusion pharmacy operations Familiarity with EMR and pharmacy management systems Work Environment Pharmacy cleanroom and office setting May require occasional on-call or after-hours support Fast-paced environment with a focus on patient-centered care Physical Requirements Ability to stand for extended periods Manual dexterity for compounding sterile products Ability to lift and move supplies as needed Why Join Us Opportunity to provide high-impact, patient-centered care in the home setting Collaborative and supportive clinical environment Growing field with strong professional development opportunities
