Salt Lake City, UT 84116 Gunnison, UT 84634
SCHEDULE:
As Needed
QUALIFICATIONS
Must maintain all licenses required to lawfully perform the Pharmacist services and ensure they remain current. Maintain current, active, and unrestricted licenses in the State of Utah applicable to pharmacists Must submit proof of board certification or board eligibility in the Pharmacia field. Maintain a valid Controlled Substance License and Drug Enforcement Administration certification with no disciplinary actions.
JOB DUTIES
Prepare medications according to prescriptions written by authorized medical practitioners; Provide drug information and consultation to physicians, nurses, patients, and other clinical staff; Order and maintain a secure inventory of drugs, chemicals, and controlled substances; Fill and refill prescriptions as authorized; Prepare reports on medication and drug usage patterns; and Perform other pharmaceutical tasks assigned by DHHS staff. #IND1
