Schedule may fall between 7:00 AM-11:00 PM; exact schedule to be discussed during interview We are seeking an experienced Inpatient Pharmacist to join a hospital-based team. This role is ideal for a pharmacist with recent acute care experience who is comfortable working independently and participating in a highly autonomous pharmacist-as-practitioner model. Responsibilities Provide inpatient pharmacy services in an acute care hospital setting Perform complex medication reconciliation and make medication/order recommendations Make medication-related changes in collaboration with the hospitalist team Provide pharmacotherapy management and clinical pharmacy support Work independently while collaborating with physicians and other healthcare professionals Support safe and effective medication use across a high-volume inpatient environment Requirements Active Virginia pharmacist license and good professional standing PharmD or B.S. in Pharmacy Recent inpatient/acute care pharmacy experience required Epic experience required Strong clinical and pharmacotherapy knowledge IV/IV medication experience Ability to work independently in a high-autonomy practice model Excellent communication and interpersonal skills Preferred Qualifications Pharmacy residency from an accredited ASHP institution Experience in a computerized hospital pharmacy setting Pyxis experience Carousel system experience Additional Details Average hospital census: 330-360 No on-call requirement Weekend and holiday rotation required First-time travelers/clinicians are welcome to apply Local candidates accepted Block scheduling may be considered based on manager approval If you have recent acute care experience and are interested in an autonomous inpatient clinical pharmacy role, we encourage you to apply.
