•$85.92 per hour The OP Ambulatory Pharmacist Clinician Primary Care is an advanced practice pharmacist responsible for promoting optimal drug use outcomes in high risk, high cost or complex patient populations. They apply specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Participates as a contributing member of the pharmacy care team serving primary care such as but not limited to family medicine and internal medicine. Provides clinical, pharmacy, and medication expertise in each area including policy development, clinical pharmacy operations, and medication therapy outcomes.
EDUCATION
Doctor of Pharmacy from an ACPE accredited college of pharmacy Licensed pharmacist in the State of Vermont and New York PGY-1 Pharmacy Residency encouraged PGY-2 Pharmacy Residency preferred Board Certified by Board of Pharmacy Specialties or board eligible within 4 years of employment
EXPERIENCE
3 years of advanced practice experience preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
KNOWLEDGE/SPECIAL SKILLS
Excellent written, verbal, and interpersonal skills required. Excellent organizational and team building skills. Ability to work effectively as part of a clinical care team. Ability to exercise sound professional judgement, set priorities, make critical decisions and respond quickly to emergent requests. Licensed pharmacist in the State of Vermont and New York. This position requires a high degree of specialized knowledge of clinical therapeutics within their area of expertise. Frequently applies a high degree of analytical, interpretive and evaluative skills as part of clinical care responsibilities. Apply
