Job Description
- The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team. (2) Population Management
- The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization. Providence caregivers are not simply valued

Required Qualifications:
Bachelor's Degree inPHARMACY
from an accredited School of Pharmacy or PharmD inPHARMACY
from an accredited School of Pharmacy.Coursework/Training:
Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.Upon hire:
Washington Pharmacist License. Within 2 years of hire: Certification from Board of Pharmacy Specialties.Upon hire:
Board of Pharmacy Preceptor License- when eligible and where applicable.
