The Long-Term Care Pharmacist is responsible for ensuring the safe, effective, and appropriate use of medications in long-term care facilities, assisted living communities, and behavioral health & rehabilitation organizations. This role includes reviewing medication orders, consulting with healthcare staff, and ensuring compliance with federal and state pharmacy regulations.
Responsibilities:
Medication Review & Dispensing:
o Verify and dispense prescriptions accurately. o Review patient medication regimens for safety, efficacy, and potential drug interactions. o Monitor for contraindications, duplications, and adverse drug reactions.
Consultation & Collaboration:
o Provide drug information and clinical recommendations to physicians, nurses, and caregivers. o Attend interdisciplinary care team meetings when necessary. o Serve as a resource for medication therapy management.
Regulatory Compliance & Documentation:
o Ensure adherence to state and federal regulations, including CMS and DEA standards. o Maintain proper records of medication dispensing and patient profiles. o Participate in audits and quality assurance programs.
Inventory & Operations:
o Manage medication inventory and ensure proper storage and labeling. o Assist in pharmacy operations, including workflow optimization and policy development.
Qualifications:
Licensure:
o Active and unrestricted pharmacist license in Washington state.
Experience:
o Preferred 1-2 years of experience in long-term care, geriatrics, or institutional pharmacy. o Excellent clinical skills and experience in MTM. o Experience with the FrameworkLTC and FrameworkECM software platform is ideal. o Understanding of billing, processing and delivering LTC prescriptions.
Skills:
o Strong clinical knowledge in geriatric pharmacotherapy. o Excellent communication and problem-solving abilities. o Ability to work independently and collaboratively in a team-oriented environment. o Excellent computer skills and curiosity about technology and a passion for change
Pay:
$65.00 - $75.00 per hour
