Qualifications:
Active Pharmacist license in the state of Wisconsin PharmD or BS Pharmacy degree required 1+ year of previous experience in Home Health Or Hospice pharmacy setting Strong clinical judgment and attention to detail
Pharmacist Contract Details:
$2,120 - $2,286 per week
• AM shift available 13-week contract with possibility to extend
Key Responsibilities:
Medication preparation and dispensing Review and interpret medication orders for accuracy and safety Monitor patient medication therapy and adjust as needed Ensure compliance with all state, federal, and facility regulations Maintain accurate records and documentation Support pharmacy operations and workflow as needed
