Job Description

Job Summary Provides clinical and distributive pharmacy services to infant, pediatric, adolescent, adult, and geriatric patients. Incorporates educational, clinical, and technical pharmacy functions to prepare medications, assess and monitor medication therapy. Assists facility staff with medication-related issues and provides drug information to nursing and medical staff. Responsibilities Service Provides accurate and timely drug information to practitioners and the professional staff. Communicates appropriate pharmaceutical information clearly, professionally, and concisely. Consults with practitioners and professional staff as needed to clarify or to ensure positive therapeutic outcomes and to minimize drug-related problems/errors. Labels and dispenses drugs accurately and completely, and in a timely manner. Reviews patient profiles for any interactions based on diagnosis, allergies or duplications via order entry and appropriately responds to electronic alerts. Ensures dosing is appropriate for patient's weight and age. Performs pharmacokinetic monitoring according to protocol, including appropriate ordering and interpretation of labs and ordering of drug regimens. Monitors medication therapies identified in the pharmacy department policy. Safety and Quality Educates patients and family members, on an as needed basis. Education is appropriate for age, cultural and psychosocial needs, and living conditions of the patient and/or family. Participates in the quality improvement and medication use review activities of the department. Conducts effective literature evaluations. Demonstrates appropriate sterile aseptic technique, order of mixing, use of compounding equipment and other pharmaceutical preparation practices. Ensures that controlled substances and corresponding records are handled appropriately. Verifies accuracy of all products prepared for patients by technicians. Leadership Supervises the duties of pharmacy technicians performed during shift. Directs and assists pharmacy technicians in the completion of tasks. Demonstrates the ability to make critical decisions in a timely manner. Adapts positively to frequent interruptions and changes in workload and/or work schedule. Assists in the orientation of new pharmacy employees and pharmacy students for experiential rotations. Assists with special projects as requested. Financial Sustains and assists in management of the formulary. Demonstrates concise, accurate and complete documentation for all patient- specific records, following established standards of practice.
Knowledge, Skills & Abilities Medium Work:
Occasional lifting and/or carrying of objects weighing up to 25 lbs with a maximum lifting of 50 lbs. Training in proficiency of age-specific therapies for ages served by the pharmacy. Education Bachelor's Degree (Required) Doctorate Degree (Required) Credentials Pharmacist, Licensed (Required)
