Pharmacy Practice(Population Health/ Ambulatory Care) The University of Maryland Eastern Shore (UMES) seeks a full-time, twelve-month, non-tenure track faculty position. UMES is located in rural Maryland and is one of America's most beautiful campuses. Salary:

$116,500 - $135,960

Responsibilities:

• Provide didactic and experiential instruction in pharmacy practice, withemphasis ambulatory care and related disciplines.
• Maintain an active clinical practice that supports interprofessionalcollaboration and student learning.
• Develop, deliver, and continuously improve courses to meet and exceedaccreditation standards; collaborate effectively in team-taught courses.
• Engage in independent and collaborative scholarly activities related to areaof expertise.
• Participate in service to the school/university and regional, state, andnational professional organizations.
• Provide comprehensive population health/ambulatory care pharmacyservices, including comprehensive medication management, chronicdisease management, medication reconciliation, transitions of care, andpatient education to optimize patient outcomes.
• Collaborate with physicians, care managers, and other healthcareprofessionals to identify and resolve medication-related problems, improveadnce, close care gaps, and advance population health initiativesthrough evidence-based care and quality improvement activities.
• Utilize electronic health records, population health tools, and clinical qualitymetrics to identify high-risk patients, document and assess pharmacistinterventions, and contribute to the development and implementation ofinnovative pharmacy services and practice protocols.
• Perform other related duties as assigned.

Required Minimum Qualifications:

• Doctor of Pharmacy required.
• Completion of a post-graduate year one (PGY1) residency preferred or at least three (3) years of experience in clinical pharmacy practice or ambulatory care.

• Excellent communication skills (verbal and written) and organizational skills.
• A team approach to patient care.
• Strong commitment to teaching excellence, student engagement, research collaboration, and service.

Preferences:

• Experience in didactic and experiential teaching. Completion of a post- b graduate year two (PGY2) ambulatory care residency.
• Board certification in ambulatory care.
• Experience in population health.

Physical Demands:

• May require extended periods of standing, bending, stooping, sitting at desk.
• May require lifting.
• Requires communication with a variety of constituents externally and internally.
• Requires operation of a variety of office equipment.

Licenses/ Certifications:

• Current Maryland pharmacy license or eligibility for Maryland licensure required.
