We are seeking a Care Manager with pharmacy technician experience to support patients receiving ongoing medication therapy. This is a patient care coordination role, not a dispensing position . The ideal candidate understands prescription workflows, refill schedules, prior authorizations, medication terminology, and pharmacy processes and enjoys working directly with patients. Responsibilities Provide ongoing patient outreach and care coordination Track prescriptions, refills, prior authorizations, and medication delivery Coordinate with physician offices, pharmacies, and clinical teams Help patients follow prescribed medication and treatment schedules Identify medication-related questions or concerns and escalate them to the appropriate licensed clinician Document patient interactions and follow-up activities Track outstanding items and ensure timely follow-through Qualifications Pharmacy Technician experience preferred Specialty pharmacy or prior authorization experience is a plus Strong patient communication and customer-service skills Excellent organization and follow-through Comfortable working with healthcare technology and multiple patient cases This is an excellent opportunity for a pharmacy technician interested in moving into a patient-focused care management role.

Pay:

$18.00 - $21.00 per hour
