Job Description

Certified Medical Assistant / Phlebotomist - Hybrid Evercare Health at Home LLC Upstate South Carolina Full-Time |

Hybrid:

Work From Home + Field-Based Patient Care Evercare Health at Home LLC is seeking an experienced, dependable Certified Medical Assistant (CMA) / Phlebotomist to join our growing home-based primary care practice serving patients throughout Upstate South Carolina. This is a unique hybrid position combining work-from-home clinical support and care coordination with in-person visits to patients in their homes and residential communities . The ideal candidate is comfortable working independently, enjoys caring for older adults and medically complex patients, and has strong phlebotomy and documentation skills. Schedule This is a full-time hybrid position , generally consisting of: 2-3 days per week working from home 2-3 days per week traveling to patient homes and residential communities throughout Upstate South Carolina Monday-Friday schedule Daytime hours Responsibilities Field / Patient Care Responsibilities Travel to patients' homes, assisted living communities, and other residential settings Perform venipuncture and specimen collection Prepare and properly label laboratory specimens for processing Obtain vital signs and other clinical measurements Assist with point-of-care testing and other clinical procedures within scope Support the provider during home-based primary care as needed Communicate changes in patient condition to the provider Maintain appropriate infection-control and specimen-handling procedures Provide compassionate care to elderly, homebound, and medically complex patients Work-From-Home Responsibilities Document clinical information in Athena EMR Assist with Chronic Care Management (CCM) documentation and patient outreach Assist with Remote Patient Monitoring (RPM) documentation and follow-up Review and document patient-reported vital signs and monitoring information Contact patients and caregivers regarding care needs, appointments, and follow-up Assist with medication reconciliation, clinical messages, and care coordination Coordinate laboratory orders and results Communicate with home health agencies, pharmacies, specialists, facilities, and other members of the patient's care team Support ongoing primary care operations and provider workflow

Qualifications Required:

Current Certified Medical Assistant (CMA) credential or equivalent recognized medical assistant certification Phlebotomy experience with strong venipuncture skills Valid driver's license Reliable personal vehicle and ability to travel throughout Upstate South Carolina Current automobile insurance Ability to work independently in patients' homes Strong communication and documentation skills Comfortable working with elderly and medically complex patients Reliable internet connection for work-from-home responsibilities Ability to use electronic medical records and other web-based clinical platforms

Preferred:

Experience performing mobile or in-home phlebotomy Experience in home-based primary care, home health, hospice, palliative care, or mobile healthcare Experience working with geriatric patients Experience with Athenahealth / Athena EMR Experience with CCM and RPM programs and documentation Experience working with patients in assisted living facilities, group homes, or other residential settings Experience coordinating care with home health agencies and outside providers Travel Requirements This position requires regular travel throughout Upstate South Carolina . Employees must use their own reliable vehicle for patient visits. Business mileage is reimbursed.
