2 Description American Traveler is looking for a Home Health Physical Therapist with 1+ years of recent geriatric home health experience and HCHB proficiency. Details Home Health setting treating geriatric patients Day shift schedule, 5x8-hour shifts, 8:00 AM to 5:00 PM Average caseload of 5-7 patients per day with a minimum weekly productivity of 30 points Territory covers up to a 60-mile radius from the hub office No on-call requirement, though rotation may be discussed during interview Requirements Minimum 1 year of recent Home Health PT experience required BLS certification required License required if applicable License must be in-hand at time of consideration Proficiency in HCHB (Homecare Homebase) EMR required OASIS experience required for staff completing Start of Care (SOC) assessments Active driver's license and personal transportation required Additional Information Responsibilities include timely communication and documentation of patient care Monday orientation required, including completion of billable training modules Scrubs required in any color; no jeans permitted iPad provided for documentation; personal cell phone use not permitted Mileage reimbursed at the IRS standard rate First-time travelers will not be considered for this role Locals accepted at the travel rate Travel pairs accepted, though each traveler is assigned an individual territory and must have their own transportation Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler Requested time off (RTO) must be submitted at the time of application Completion of a Hospice/Home Health checklist is mandatory
