Position Overview The Physical Therapist assesses, teaches, and delivers therapy interventions based on physician-directed home health plans. They collaborate with an interdisciplinary team, providing updates to optimize patient well-being.
Key Responsibilities:
Perform assessments of mobility, strength, and function. Administer, evaluate, and interpret tests to set functional goals. Develop and update plans of care, documenting progress and outcomes. Deliver therapy services according to physician orders. Coordinate discharge planning and educate patients and caretakers on home exercises. Collaborate with healthcare team members, document effectively, and supervise personnel if applicable. Complete assessments like OASIS where permitted.
Qualifications & Skills:
Degree in Physical Therapy from an accredited program. Current, unrestricted state license. Proficiency in assessments, documentation, and policy adherence.
Preferred Attributes:
Strong organization, communication, and attention to detail. Effective time management and relationship-building skills. Experience in home health therapy and OASIS documentation is a plus.
