• Skilled Nursing Facility for an exciting Travel or Local job in Phoenix, AZ.

Key Responsibilities Evaluate patients and develop individualized physical therapy treatment plans Provide therapeutic exercises, mobility training, and rehabilitation services to patients recovering from injury, illness, or surgery Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals Educate patients and family members on therapy programs, exercises, and injury prevention techniques Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient care Maintain accurate and timely documentation of patient treatments, progress notes, and discharge planning Assist patients in improving strength, balance, flexibility, mobility, and functional independence Ensure compliance with facility policies, patient safety standards, and healthcare regulations Participate in care planning meetings and contribute to quality patient outcomes Required Qualifications Active Physical Therapist (PT) license in the state of Arizona or compact eligibility Graduate of an accredited Physical Therapy program Current CPR/BLS certification preferred Minimum of 1 year of recent Physical Therapist experience preferred Skilled Nursing Facility (SNF) or rehabilitation experience required Strong communication, patient care, and clinical assessment skills For more details contact at or Call / Text at 516-862-1169.
