What You'll DoPerform comprehensive physical therapy evaluations and establish individualized plans of care.

Develop and implement treatment plans to improve mobility, strength, balance, and functional independence.

Monitor patient progress and modify treatment plans to achieve optimal outcomes.

Educate patients, families, and caregivers on exercises, safety, and disease management.

Collaborate with physicians, nurses, and the interdisciplinary care team to coordinate patient care.

Complete accurate and timely clinical documentation.

Maintain compliance with infection control standards, agency policies, and regulatory requirements.

QualificationsGraduate of an accredited Physical Therapy program.

Current, unrestricted Physical Therapist (PT) license.

Current CPR certification.

Valid driver's license, reliable transportation, and current auto insurance.

One (1) year of Home Health experience preferred.

HomeCare HomeBase experience preferred.

Physical RequirementsAbility to stand, walk, bend, squat, kneel, climb, and sit for extended periods.

Ability to safely lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
