Flexible PRN Schedule | Home Health | Make an Impact One Patient at a Time Are you a Physical Therapist looking for flexibility, independence, and meaningful patient care? Join our Home Health team as a PRN Physical Therapist and provide personalized rehabilitation services to patients in the comfort of their homes. In this role, you will assess, plan, organize, and provide rehabilitative services designed to improve mobility, relieve pain, increase strength, restore function, and help patients maintain their independence. Physical Therapist, You Will:
Perform and document comprehensive initial evaluations to identify functional limitations and determine appropriate treatment interventions. Develop and implement individualized physical therapy treatment plans based on each patient's needs and goals. Provide therapeutic interventions designed to improve mobility, strength, balance, function, and overall quality of life. Evaluate patient progress and modify treatment plans as necessary to achieve optimal outcomes. Apply infection control and universal precaution standards during all patient care activities. Ensure continuity and quality of patient care through timely and accurate documentation. Communicate effectively with patients, families, physicians, caregivers, and other members of the interdisciplinary care team. Educate patients and caregivers regarding treatment plans, exercises, safety, and strategies to promote independence. Maintain and complete all required documentation within company-established guidelines. Promote the agency's philosophy, policies, and commitment to quality patient care. Requirements Graduate of an accredited Physical Therapy program. Current and valid Physical Therapist license in the state of practice. CPR certification. Valid driver's license. Current automobile insurance. 1 year of Home Health experience preferred. HomeCare HomeBase (HCHB) experience preferred. Physical Requirements Ability to stand, walk, sit, climb, crawl, squat, or kneel as required while providing patient care. Ability to lift, carry, push, or pull up to 100 lbs. , with appropriate assistance or equipment when necessary. PRN Perks Flexible PRN scheduling.
