Physical Therapist - Home Health (Full-Time / Part-Time)

Full-Time / Part-Time Job Summary Primamed Health Services Inc. is seeking a compassionate and experienced Physical Therapist (PT) with home health experience to join our growing team. The Physical Therapist will be responsible for evaluating patients, developing individualized treatment plans, conducting skilled therapy visits, and completing patient discharges in the home setting. The ideal candidate must have strong clinical skills, excellent communication, and experience working independently in home health care. Responsibilities Perform initial physical therapy evaluations in patients' homes. Develop individualized treatment plans based on patient needs and physician orders. Provide skilled physical therapy visits to improve mobility, strength, balance, and functional independence. Assess patient progress and modify treatment plans as necessary. Complete reassessments, recertifications, and discharge evaluations. Educate patients and caregivers regarding exercises, safety precautions, fall prevention, and home programs. Coordinate care with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals. Maintain accurate and timely documentation in the electronic medical record system. Ensure compliance with home health regulations, agency policies, and Medicare guidelines. Participate in case conferences and interdisciplinary team meetings as needed. Qualifications Current California Physical Therapist license in good standing. Minimum of 1 year home health experience preferred. Knowledge of OASIS documentation and Medicare home health guidelines is preferred. Current CPR certification. Valid California driver's license and reliable transportation. Excellent assessment, communication, and time management skills. Ability to work independently and manage patient schedules efficiently.
