Core experience in Orthopedic & Sports Rehab, Therapeutic Exercise Design, Neurological Rehabilitation, Chronic Pain Management, Gait & Balance Training, and Chronic Pain Management preferred. Job Overview We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a strong background in physiotherapy, with experience in outpatient, inpatient, and home health settings. This role involves developing personalized treatment plans, collaborating with healthcare professionals, and delivering exceptional patient care to promote recovery and improve quality of life across diverse patient populations. Responsibilities Assess patients' physical conditions through detailed evaluations, including physiology and anatomy knowledge, to determine appropriate treatment strategies. Develop and implement individualized care plans that align with clinical documentation standards and best practices in physiotherapy. Deliver effective therapeutic interventions such as manual therapy, trigger point therapy, and rehabilitation exercises tailored to patient needs. Collaborate closely with physicians, nurses, and other healthcare professionals to coordinate discharge planning and ensure continuity of care. Manage electronic health records (EHR) systems accurately for documentation of treatment progress and patient outcomes. Provide education and guidance to patients and their families regarding home & community care routines to support ongoing recovery. Monitor patient progress regularly, adjusting treatment plans as necessary to optimize rehabilitation outcomes in outpatient clinics, hospitals, or home health environments. Requirements Valid licensure or certification as a licensed Physical Therapist in the respective jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or inpatient settings. Strong knowledge of physiology, anatomy, neurology, orthopedics, pediatrics, geriatrics, and related medical terminology. Experience with outpatient rehabilitation center operations. Familiarity with clinical documentation standards and EMR (Electronic Medical Records) or EHR (Electronic Health Records) management systems. Ability to work effectively with geriatric patients and those requiring specialized therapies such as trigger point therapy. Excellent client communication skills combined with the ability to collaborate seamlessly with multidisciplinary healthcare teams. Prior experience in discharge planning, acute care environments, or inpatient hospital settings is highly desirable. Travel may be required to nearby centers.
