Job Description

Lead Physical Therapist - Memory Care Prava Therapy - 4.2 Carlsbad, CA Job Details Full-time $52 - $60 an hour 1 hour ago Benefits Opportunities for advancement Qualifications Teamwork Physical Therapy Interpersonal skills Collaboration Physical Therapy License Full Job Description Lead the care. Build the program. Become part of the community. At Prava , we believe the best therapy programs are built around exceptional clinicians—not productivity quotas and paperwork. We're looking for a Full-Time Lead Physical Therapist to become the clinical leader of therapy within a senior living memory care community. You'll provide meaningful one-on-one care while taking ownership of the therapy program, building relationships with residents and families, collaborating with community leadership and care teams, and helping create a standard of rehabilitation that truly improves residents' lives. This Isn't Traditional Therapy You won't be expected to see 18-20 patients every day. You won't be racing between treatments or double-booking patients. You won't be taking hours of documentation home at night. Instead, you'll have the opportunity to lead a therapy program within one community . You'll become someone families know and trust. Caregivers will rely on your expertise. Nurses will seek your clinical judgment. Community leaders will view you as a partner. Other clinicians will look to you for leadership. And most importantly, residents will receive consistent care from someone who truly knows them. What You'll Lead As the Lead Physical Therapist, you'll balance direct patient care with clinical and program leadership.
You'll:
Provide high-quality, one-on-one physical therapy to residents. Serve as the primary therapy leader within the community. Build strong relationships with residents, families, caregivers, nursing staff, and community leadership. Collaborate with community staff to identify residents who may benefit from therapy. Help develop fall-prevention, mobility, wellness, and functional independence initiatives. Support and mentor other therapy team members as the program grows. Help establish clinical standards and best practices. Participate in interdisciplinary care planning. Serve as a trusted rehabilitation resource for the entire community. Help grow a therapy program focused on exceptional outcomes rather than volume. Less Paperwork. More Leadership. More Purpose. Prava's AI-powered clinical platform automates much of the documentation and administrative work that traditionally consumes a therapist's day.
That means more time for:
Patient care. Clinical decision-making. Team collaboration. Program development. Leadership. And less time clicking through unnecessary administrative tasks. Why Memory Care? Because few settings give a therapist the opportunity to make such a meaningful impact. You'll help residents: Walk with greater confidence. Reduce their risk of falls. Maintain mobility and independence longer. Remain engaged in meaningful daily activities. Preserve functional abilities as their needs change. Improve their overall quality of life. Support families and caregivers navigating dementia-related changes. Success isn't always measured by dramatic changes on an outcome measure. Sometimes it's helping someone continue walking to the dining room. Sometimes it's preventing the next fall. Sometimes it's helping a resident participate in an activity they love. And sometimes it's giving a family more confidence that their loved one can remain independent. A Leadership Opportunity With Room to Grow We're looking for someone who doesn't just want to fill a position—we want someone who wants to build something . As the therapy program grows, you'll have opportunities to mentor clinicians, help develop new programs, influence clinical operations, and grow within Prava's leadership structure.
Potential career pathways include:
Clinical Mentor Multi-Site Therapy Leadership Program Development Practice Leadership Regional Clinical Leadership New Graduate With Leadership Potential? Ask Us About ASCEND. Our ASCEND Mentorship Program was built for therapists who want more than a traditional orientation. You'll receive: One-on-one mentorship from experienced clinicians. Clinical coaching and case consultation. Specialized memory care and geriatric education. Leadership development. Support treating complex patient populations. A clear pathway for continued professional growth. We believe great clinical leaders are developed—not simply hired. What You'll Love Full-time, consistent position within one community. Smaller daily caseloads focused on quality over quantity. Meaningful one-on-one patient relationships. No double-booking. Dedicated leadership responsibility and autonomy. AI-powered documentation and workflow support. Strong collaboration with community leadership and care teams. Competitive compensation and benefits. Continuing education and professional development support. Opportunities for advancement as Prava grows. We're Looking For Someone Who... Is a licensed Physical Therapist or eligible for PT licensure. Wants to take ownership of a therapy program. Loves building relationships with patients, families, and interdisciplinary teams. Is comfortable taking initiative and solving problems. Enjoys mentoring and supporting other clinicians. Wants to practice at the top of their license. Has an interest in geriatrics, dementia care, fall prevention, and functional mobility. Values quality of care over productivity quotas. Is excited about combining clinical care, leadership, and innovation. Experience in senior living, geriatrics, dementia care, or clinical leadership is valuable, but leadership potential and the right mindset matter just as much. Our Promise No double-booking. No assembly-line therapy. No sacrificing quality for unrealistic productivity expectations. Instead, we're giving clinicians the technology, autonomy, and time they need to provide exceptional care—and giving our Lead Therapists the opportunity to build programs they're proud to lead. In memory care, we're not trying to change a diagnosis. We're helping people preserve their independence, protect their dignity, reduce preventable decline, and create more meaningful moments with the people they love. If you're looking for a position where you can treat patients, lead a team, build a program, and make a lasting impact on an entire community , we'd love to meet you. Come help us redefine rehabilitation.
