Full Job Description Job Summary :
The Physical Therapist (PT) is responsible for assessing and providing physical therapy services to patients, aimed at improving mobility, function, and overall physical well-being. The PT will design and implement individualized therapy plans in compliance with Title 22, the California Business and Professions Code, and other applicable state and federal regulations.
Key Responsibilities:
Patient Evaluation and Assessment :
Conduct comprehensive physical therapy assessments to evaluate patients' physical condition, functional abilities, and limitations. Review patient medical records, including diagnoses, lab results, and physician orders, to inform the assessment and therapy plan. Assess patients' mobility, strength, balance, coordination, range of motion, and pain levels.
Develop and Implement Treatment Plans :
Develop individualized treatment plans based on physical assessments, medical diagnoses, and patient goals. Implement therapy interventions to address the patient's needs, including exercises, manual therapy, mobility training, and pain management techniques. Modify treatment plans as necessary based on patient progress and changing conditions, ensuring the plans remain in compliance with Title 22 and other relevant regulations.
Patient Education and Training :
Educate patients and their caregivers on exercises, techniques, and lifestyle modifications that can help manage and improve their physical condition. Provide training on the use of assistive devices, including wheelchairs, walkers, and prosthetics, to improve mobility and independence. Advise on fall prevention strategies and the safe performance of daily activities to reduce risk of injury. Collaboration with
Healthcare Team :
Collaborate with physicians, nurses, dietitians, social workers, and other members of the interdisciplinary team to integrate physical therapy goals into the patient's overall care plan. Participate in regular care team meetings to discuss patient progress, adjust goals, and address any concerns related to physical therapy interventions. Communicate effectively with team members regarding patient status, changes in condition, and treatment recommendations.
Compliance and Regulatory Adherence :
Ensure all physical therapy services are delivered in compliance with Title 22 of the California Code of Regulations, including the provision of appropriate documentation and adherence to staffing ratios. Adhere to all state and federal regulations regarding physical therapy services, including the California Physical Therapy Practice Act and applicable health and safety codes. Document therapy sessions, patient progress, and any changes in treatment plans in patient records, in accordance with facility policies and Title 22 requirements.
Quality Assurance and Improvement :
Participate in quality assurance and performance improvement initiatives to enhance the effectiveness of physical therapy services and overall patient care. Conduct regular reviews of patient outcomes to assess the effectiveness of treatment interventions and adjust plans as necessary. Maintain and review physical therapy equipment to ensure it is properly maintained and compliant with safety standards.
Documentation and Record Keeping :
Maintain accurate, up-to-date patient records, including therapy progress notes, treatment plans, assessments, and outcomes. Ensure that all documentation complies with state and federal regulations, including the Health Insurance Portability and Accountability Act (HIPAA). Prepare and submit required reports and documentation to the ADHC Program Director, regulatory bodies, or other stakeholders as necessary.
Staff Education and Support :
Provide training and education to other staff, including caregivers and interdisciplinary team members, on techniques to support patient mobility and physical function. Offer guidance to staff in the safe handling and transfer of patients, as well as fall prevention and mobility assistance strategies. Mentor and supervise physical therapy assistants (PTAs) or other support staff, ensuring they adhere to clinical protocols and provide quality care.
Qualifications:
Education :
Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program. Completion of required clinical internships or residency programs in physical therapy.
Licensure :
Current and valid Physical Therapist license issued by the California Department of Consumer Affairs, Physical Therapy Board of California. Knowledge of and compliance with the California Physical Therapy Practice Act and Title 22 regulations.
Experience :
Minimum of 1 year of experience as a Physical Therapist, preferably in an ADHC, rehabilitation, or long-term care setting. Experience in treating older adults and individuals with chronic medical conditions, including neurological, orthopedic, and musculoskeletal disorders.
Skills :
Strong clinical skills in physical therapy assessment, diagnosis, and treatment techniques. Ability to develop and implement individualized treatment plans and adapt to changes in patient conditions. Excellent communication and interpersonal skills, with the ability to work collaboratively in an interdisciplinary team. Organizational and documentation skills, with attention to detail to ensure compliance with regulatory standards. Certifications (preferred): Certification in specialized therapy areas such as Geriatrics, Neurology, or Orthopedics (e.g., GCS, NCS). Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification.
