Home Health Physical Therapist (PT) Per diem

As we continue expanding across Fresno County and surrounding communities , we are seeking a compassionate and skilled Physical Therapist to join our dynamic Home Health team. Qualifications:

Current California Physical Therapy License in good standing. Minimum 1 year of experience in Physical Therapy (Home Health or Hospice experience preferred but not required). Strong communication, clinical documentation, and time management skills. Valid driver's license and reliable transportation for daily field visits. Up-to-date CPR certification and TB clearance .

Preferred Qualifications:

Experience in Home Health or Hospice settings. Comfort working with diverse patient populations and tailoring treatment plans accordingly. Why Join InHome Healthcare Services? Key Responsibilities:

Provide in-home physical therapy services and assess patient conditions. Monitor and document vitals, mobility, functional abilities, and therapy progress. Recommend therapeutic interventions, assistive devices, and safety modifications. Collaborate with RNs, OTs, physicians, and care team members to develop personalized care plans. INDFR Pay:

$55.00 - $65.00 per hour
