Home Health Physical Therapist Equi Care | East Bay, CA |
Full-time Pay:
$120,000-$150,000/year Equi Care is hiring a licensed Physical Therapist to provide in-home rehabilitative care to patients throughout the East Bay, helping them regain mobility and stay safely independent at home. Responsibilities Evaluate patients in-home and build individualized treatment plans Provide therapeutic exercise, gait training, and fall-risk reduction Educate patients/caregivers on home exercise and safety Document visits per Medicare/agency standards Coordinate care with physicians, nurses, and case managers Requirements DPT degree and current CA PT license Current CPR/BLS certification Valid driver's license, reliable vehicle, and insurance Home health or geriatric experience preferred
