Responsibilities Assess patients' physical conditions by conducting comprehensive evaluations focusing on mobility, strength, and functional abilities. Develop and implement individualized treatment plans aligned with clinical best practices and patient goals. Deliver evidence-based physiotherapy interventions including manual therapy, trigger point therapy, and therapeutic exercises. Monitor patient progress regularly and adjust treatment strategies to optimize recovery outcomes. Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care plans. Assist in discharge planning by preparing patients for transition to home or community care settings, ensuring continuity of care. Requirements Valid license to practice as a Physical Therapist in the relevant jurisdiction with current practicing credentials. Proven experience in outpatient rehabilitation centers or hospital inpatient units, with strong knowledge of physiotherapy techniques. Familiarity with medical terminology, anatomy, physiology, and neurology to inform treatment development. Hands-on experience working with geriatric patients, pediatrics, or neurological conditions is highly desirable. Proficiency in electronic health records (EHR) and electronic medical records (EMR) systems for accurate documentation and reporting. Demonstrated ability to develop effective care plans tailored to individual patient needs across diverse populations including home health and outpatient settings.
