Job Description
South Pacific Rehab Services seeks a licensed PTA to provide hands-on physical therapy interventions in skilled nursing and post-acute care settings across Hawaii. Working under the direction of a PT, you will implement individualized treatment plans, assist with therapeutic exercises, gait and balance training, and functional mobility to maximize independence and quality of life. You will document progress, collaborate closely with an interdisciplinary rehab team, educate patients and families, and support discharge planning in a respectful, patient-centered environment with strong mentorship and growth opportunities. Responsibilities Implement PT treatment plans under direction of a licensed Physical Therapist Provide therapeutic exercises, gait, balance, and functional mobility training Assist with transfers, safety, and fall prevention for post-acute and long-term care patients Accurately document treatments, progress, and patient responses in EMR systems Collaborate with PTs, OTs, nursing, and other rehab team members on care plans Educate patients and families on exercises, safety, and functional strategies Support discharge planning and functional goal achievement Maintain compliance with state licensure standards and facility policies Required Skills Physical therapy interventions Therapeutic exercise Gait and balance training Functional mobility training Rehab documentation Electronic medical records Patient and family education Interdisciplinary team collaboration Post-acute/Skilled nursing rehab Safety and transfer techniques
