Duties Conduct comprehensive assessments of patients' physical conditions, utilizing your knowledge of physiology and anatomy to develop tailored care plans. Educate patients and their families on injury prevention, mobility exercises, and self-care strategies to support long-term wellness. Maintain accurate documentation of patient progress using Electronic Medical Record (EMR) systems while adhering to medical terminology standards. Assist in the management of acute care cases by implementing targeted therapies that address immediate needs while considering overall care plans. Participate in ongoing professional development to stay current with advances in physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Demonstrated experience working in home health environments. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy principles. Proven ability to develop and implement effective care plans for diverse populations including pediatrics and geriatrics. Experience with discharge planning processes and coordinating patient care across multiple settings. Familiarity with EMR systems for documentation and patient record management. Excellent communication skills to educate patients effectively and collaborate with healthcare teams.
