Physical Therapist Position Compensation:
Hourly and Paid Per Visit (PPV) hybrid model (negotiable based on experience, skill level, availability, etc.) Opportunity to accumulate overtime Essential Job Functions / Responsibilities Provides physical therapy services to patients according to a physician's written plan of care and as defined in the state Physical Therapy Practice Act.Responsibilities may include:
Assessing and evaluating therapeutic, rehabilitative, and functional status, including home environment Assessing muscle strength, mobility, gait, range of motion, and potential for rehabilitation Directing and implementing physical therapy treatments Instructing patients and families/caregivers in the use and care of therapeutic appliances Determining priority needs for physical therapy Reporting patient reactions to treatment or changes in condition to the physician Initiating physical therapy programs and instructing other personnel or family/caregivers as needed Training patients in the use of prosthetic devices Identifying patient and caregiver needs for other home health services and making referrals as appropriate Preparing and submitting clinical and progress summaries based on goal attainment Participating in patient discharge planning Providing physical therapy consultation to families/caregivers when indicated Providing in-service education programs for nursing personnel as needed Participating in peer consultation Supervising physical therapy assistants according to organizational policy and state regulations Instructing, supervising, and evaluating home health aide care every two weeks when therapy is the only skilled service The above statements are intended to represent the major duties and responsibilities of this job. Incumbents may be requested to perform other related tasks as needed. Position Qualifications Degree from a master's or doctoral program in physical therapy from an accredited organization Licensed to practice as a Physical Therapist in California Minimum of one year of experience as a Physical Therapist; community/home health experience preferred Strong verbal and written communication and organizational skills Current CPR certification Licensed driver with a reliable, insured vehicle in accordance with organizational requirements Compassionate Care was established in 2003 in Fresno County and has grown into one of California's most trusted providers of Home Health, Hospice, and In-Home Care services.
