Skilled Nursing Facility Assignment type: Travel Responsibilities Conduct initial patient evaluations to determine PT needs for residents in a skilled nursing setting. Develop and implement individualized treatment plans with measurable, function-focused goals. Provide evidence-based interventions (e.g., therapeutic exercise, gait and balance training, functional mobility, manual therapy as appropriate). Reassess progress, document outcomes, and adjust plans to maximize functional recovery. Collaborate with an interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to coordinate care and progress toward discharge. Educate patients and families on home exercise programs, safety, and strategies to maintain gains after discharge.
