Overview Join our dynamic outpatient rehabilitation team as an Outpatient Physical Therapist, where your expertise will directly impact patients' recovery journeys and enhance their quality of life. In this role, you will deliver personalized physiotherapy services within outpatient clinics, collaborating closely with healthcare professionals to develop effective treatment plans. Duties Assess patients' physical conditions by reviewing medical histories and conducting comprehensive evaluations focused on musculoskeletal, neurological, and geriatric needs. Develop and implement individualized treatment plans utilizing techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics-focused rehabilitation strategies. Deliver hands-on therapy sessions including therapeutic exercises, manual therapy, and patient education to promote recovery and functional independence. Manage clinical documentation standards by accurately recording treatment progress, discharge planning details, and compliance with electronic health records (EHR) systems. Conduct discharge planning to prepare patients for home or community care settings, ensuring continuity of care post-treatment. Engage in ongoing professional development to stay current with physiotherapy advancements and maintain certification requirements. Qualifications Valid licensure or certification as a licensed Physical Therapist with practicing credentials in outpatient settings. Proven experience working in outpatient clinics or outpatient rehabilitation centers with a strong focus on patient care. Demonstrated expertise in treatment plan development for diverse populations including pediatrics, adults, geriatrics, orthopedics, neurology, and home health environments. Knowledge of physiology, anatomy, medical terminology, and clinical documentation standards essential for accurate patient assessments and reporting. Familiarity with EMR (Electronic Medical Records) systems such as EMR or EHR management tools to document patient progress efficiently.
